The term accident or incident have many interpretations but broadly all definitions are the same
The term “accident” can be defined as an unplanned event that interrupts the completion of an activity, and that may (or may not) include injury or property damage.
An incident usually refers to an unexpected event that did not cause injury or damage this time but had the potential. “Near miss” or “dangerous occurrence” are also terms for an event that could have caused harm but did not.
Accident / Incident investigation and analysis is carried out in order to determine the cause or causes of an accident / incident or series of accidents / incidents. The investigation assists to identify total or partial failures of controls. It will also identify the absence of controls that should have been in place to prevent the occurrence of the event.
Analysis of information collected over a period of time could establish trends and weaknesses within the organization and it’s safety management systems.
